FULL ACCESS DETAILS
The terms of your door access are based on your plan eligibility through your health plan/employer group.
If a member becomes ineligible, their key will be turned off.
If a member has not used their key within a certain time frame it may be turned off for security purposes.
If a member has a change to their plan’s fitness benefit, see the club manager to have this updated.
ASSUMPTION OF RISK
The member expressly assumes all risks for their own health and well-being and expressly assumes all other risks associated with their participation in any Ultimate Community Health & Fitness activity. Members attest that they have informed their physician of any physical limitation and have obtained express consent from the physician to participate in physical activity.
BIKES
Bikes (both upright and recumbent) work when you begin to pedal. Before beginning a workout on a bike:
Adjust your seat to a comfortable height.
Use the foot straps to prevent slipping.
Make sure to tie your shoes, secure personal stereos (i.e., headphones, earbuds, etc.), and dangling jewelry.
CROSS-TRAINERS
Cross-trainers (both total body and lower body) work when you begin to pedal. Before beginning a workout on a cross-trainer:
Make sure to tie your shoes, secure personal stereos (i.e., headphones, earbuds, etc.), and dangling jewelry.
Keep both feet on the pedals while using.
Make sure pedals come to a complete stop before dismounting.
STRENGTH EQUIPMENT
All strength equipment is designed to perform a basic movement (e.g. Chest Press). Adjust the equipment for your size and range of movement. Before beginning a workout with our strength equipment:
Adjust the seat to your appropriate level.
Adjust the weights to your appropriate level.
Secure all loose clothing, personal stereos (i.e., headphones, earbuds, etc.), and dangling jewelry.
CARDIOVASCULAR EQUIPMENT
TREADMILLS
Treadmills allow you to walk or run in places using a belt driven by an internal motor. Before beginning a workout on a treadmill:
Make sure the treadmill belt is stationary.
Make sure to tie your shoes, secure personal stereos (i.e., headphones, earbuds, etc.), and dangling jewelry.
Straddle the belt before programming your workout or starting the treadmill.
When finished using the treadmill:
Let the belt come to a complete stop before dismounting.
NEVER attempt to mount a treadmill while the belt is running.
FREE WEIGHTS
Free weights are designed to give you a total range of motion in a movement. Free weights include dumbbells, barbells, weight plates, and rack. Before beginning a workout with our free weights:
Make sure to tie your shoes, secure personal stereos (i.e., headphones, earbuds, etc.), and dangling jewelry.
Select the weights appropriate to your level.
Abide by all posted cautions and warnings on the equipment.
NEVER sacrifice proper form to lift more weight.
NEVER use equipment that appears to be malfunctioning.
ALWAYS lift with a spotter (someone who can assist you with the weight). If you are lifting alone and during non-staffed hours, never try to lift free weights on a bench.
NEVER drop or throw weights. Use appropriate bar racks and standards.
Replace weights when finished. Weights on the floor present a tripping hazard.
SECURITY
The building and club are under 24-hour surveillance. Whether you are working out, or entering/leaving the building/restrooms, your activities are recorded. This video system is used for security purposes. The surveillance system does not protect you from harm in or on the premises of the building. A courtesy telephone is available if you feel threatened or witness suspicious activity. Do not allow access to individuals who knock on the door; doing so may put you at risk of injury or harm and could result in you losing your membership privileges. Ultimate Community Health & Fitness has unique security features. Some may be equipped with panic buttons and medical emergency call buttons. These are designed to signal authorities in the event you feel either threatened or in need of medical help.
RESTROOM AND SHOWERS
The restroom(s) have a lock that should be used when you enter the restroom. The shower(s) has a bolt lock that should be used when you enter the shower room. Please use caution when entering, using, or leaving, these areas as wet floors may cause slippage.
SEVERE WEATHER
In case of severe weather, please follow the club’s posted or stated emergency plan.
POWER OUTAGE
The door to the club has a fail-safe which will allow you to exit the club without power to the door strike. There are also emergency exit lights allowing you to see in case of a power outage.
INJURY AND FIRST AID
If you or another individual becomes injured:
For minor injuries (cuts, abrasions, etc.), a first aid kit is generally located near the lobby or office.
Abie by all posted cautions and warnings on the equipment.
Keep hands and feet away from weight stacks, moving parts, and cables while in motion. Use appropriate hand and foot grips.
NEVER sacrifice proper form to lift more weight.
NEVER use equipment that appears to be malfunctioning.
Do not try to adjust or modify the equipment with additional weight, cables, or the like.
For serious injuries that need medical treatment, dial 911 from the courtesy phone.
Report all injuries to the club by calling the number posted in the club.
FIRE
In the event of a fire, or if you smell smoke, you should exit the building immediately and call 911 from a cell phone or nearby business.
MISCELLANEOUS
The foregoing is only a general description of club policies; you must follow any other provided to you by the trainer or by any staff member or posted signage. If you have any questions or concerns about how to use the equipment, please ask a trainer or staff member, if available, or call the number posted in the club. Do not use the equipment if you are not sure how to use it or if you have any other concerns. Do not move or rearrange equipment. You should always consult with your physician before beginning any type of exercise or physical activity.

PHOTO RELEASE
MEMBER Understands MEMBER may be photographed at the CLUB, by execution of the Agreement, MEMBER agrees to allow MEMBER’s photo, video or film likeness to be used for any legitimate purpose by the CLUB and by Ultimate Community Health & Fitness, LLC and their respective producers, sponsors, organizers and/or assign, in their discretion. MEMBER acknowledges that by signing this Agreement, MEMBER gives up all Claims of ownership, income, editorial content and use of such media, and assigns all copyright ownership to Ultimate Community Health & Fitness, LLC.
TAILGAITING POLICY
Ultimate Community Health & Fitness facilities have systems in place to validate that only one person enters the facility at a time. If more than one person tries to enter the facility on one key access, an audible alarm is triggered by the tailgate system. This alarm notifies the member of someone who has entered the facility behind them. The tailgate system triggers another alarm to the video recorder, which speeds up the images-per-second on the member entrance door camera. The video recorder also creates an alarm log within itself to notify the owner that a tailgate violation has occurred and to allow for easier video review. Members are fully responsible for all actions, damage, injuries, or loss to property or person, performed by tailgated guests. Members further agree that any infractions of the club’s policies on the guest’s part may constitute loss of membership privileges. DRESS CODE
The MEMBER agrees that MEMBER shall always abide by the CLUB dress code while in the facility. Do not wear blue jeans or any pants/shorts that have blue jean type seams or rivets. Flip-flops, slides, or bare feet are not allowed in the CLUB.
COMMERCIAL ACTIVITY
The MEMBER agrees that he/she shall not engage in any type of commercial or business activity while using the facilities. MEMBER shall not act as a trainer for any other MEMBERs or guests and any acts which constitute such business activities are strictly forbidden. If the MEMBER engages in such commercial or business activities the MEMBER’s membership shall be subject to immediate cancellation.
MEMBER GUEST POLICY
MEMBER may not bring in guests at any time without the prior written consent of the facility staff. Furthermore, if this policy is violated, at the sole discretion of the facility staff, the MEMBER may have their membership suspended or canceled.
KEY FOB POLICY
The MEMBER must present upon entering the CLUB his/her membership card. The MEMBER agrees that the MEMBER may be denied access to the CLUB without his/her membership card. MEMBERS may not allow anyone else to use their key fob and must alert the CLUB if it is lost or stolen. Violating this policy may result in the suspension of your access rights and/or may be assessed to penalty fee by the facility. Members who do not have their key fob will not be allowed into the facility during non-staffed hours, nor should they enter the facility during unstaffed hours.
ADDITIONAL SERVICES AVAILABLE FOR PURCHASE
Services provided at the independent Ultimate Community Health & Fitness facility outside of the standard membership may come at an additional cost to MEMBER by independent contractors operating their own business who are retained by the CLUB. Regardless, all payments for additional services are to be made to the CLUB, who will pay the trainers as the services are provided.
CHANGE OF TERMS
Ultimate Community Health & Fitness, LLC retains the right to modify these policies without warning at any time. We may change, amend, add to, or delete any term or condition of this policy. If we make such a change, you agree that we may provide you with notice of the change by any reasonable method, such as by including an email notice and or notice to your Employer or Benefit Provider. The change will be effective upon the date of notice unless otherwise provided.
MEMBER RELEASE: I acknowledge and agree that I understand how to use all equipment, as well as all exits and security devices. I also have read and understand the above-mentioned policies and procedures about the club’s facilities and equipment and agree to abide by them. I understand the risks; including serious injury and/or death associated with using exercise equipment and exercising alone without the aid of a trainer or supervision and without the presence of club staff on the premises. I HEREBY RELEASE, INEMNIFY, AND HOLD HARMLESS ULTIMATE COMMUNITY HEALTH & FITNESS, INC., AND THE OWNERS OF ALL CLUBS WITHIN THE ULTIMATE COMMUNITY HEALTH & FITNESS SYSTEM, as well as all sponsors and advertisers, and all owners and lessors of the premises of such clubs, and their respective officers, affiliates, agents and employees WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, LOSS OR DAMAGE to person or property that may arise out of or in connection with my use of any of the equipment of the facilities of the CLUB, or any incident that occurs while using such facilities, or otherwise related to my membership. I understand that Ultimate Community Health & Fitness is independently owned and operated, and, therefore, I hereby release and agree to indemnify Ultimate Community Health & Fitness, LLC., its officers, owners, agents, employees, instructors, and their insurers from and against any liability that may arise out of or in connection with my using any of the equipment of the facilities of the club or any incident that occurs while using the club’s facilities or otherwise related to my club membership.
I expressly agree that this release is intended to be as broad and inclusive as permitted by the laws of the state and that if a portion of the release is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal effect. I understand that the owner of this facility, and Ultimate Community Health & Fitness, is relying on this release in agreeing to perform services, and I agree that this release shall be legally binding and that the terms of the release are contractual and not just mere recital. As parent or guardian of a minor, I hereby agree to be fully responsible for all actions, damages, injuries, or loss to property or person, performed by minor with or without my supervision in the club. I further agree that any infractions of the club’s policies on the minor’s part may constitute loss of minor’s and/or my membership privileges.
I have read this release and understand, and I knowingly and voluntarily sign this release. I agree to abide by all posted signage and warnings.
